Minimum Provider Contract

Ongoing monthly documentation support is subject to a $1,000 monthly minimum. The number of individuals required to meet the minimum will vary based on service type and level of documentation support.
Quarterly MAR Review Support
Starting at $150 per review cycle
Final pricing depends on the number of individuals, number of MARs reviewed, amount of documentation involved, number of errors or concerns identified, and urgency of review.
Licensing Correction & NOR Response Support
Starting at $150
Pricing begins at $150 for limited correction review. Final pricing depends on the number of corrections, number of individuals involved, amount of documentation requiring review, urgency, inspection status, policy updates needed, and level of provider readiness.
Providers with multiple correction orders, repeated inspections, contingent license concerns, altered or incomplete documentation, missing records, urgent state deadlines, or significant readiness concerns may require a higher-level correction package.
Consultation / Request Review
Initial Consultation — $75 | Up to 45 minutes
For providers who have reviewed Crown Bridge services and pricing and want to discuss fit, scope, documentation needs, or next steps.
Extended Consultation — $125 | Up to 90 minutes
For more complex documentation needs or requests requiring additional review and discussion.
A virtual meeting is not automatically required for every request. Many requests may begin by email or written request.
